Specialty: Diagnostic work-up and cognitive care planning and monitoring of adults with concerns about memory and other thinking abilities in consultation with patients' care partners. Assessment includes interview/intake where patient and family goals are identified, neuropsychological testing, and a feedback session where results, diagnostic impressions, and care plan/goals are discussed. Follow-up or cognitive care monitoring is determined on a case-by-case basis, but often 6 month check-ins help patients meet their care plan goals, and support physicians and HCPs in their care of patients with concerns about memory/cognitive loss. Caregiver coaching and a group Brain Builders program support improved self-efficacy and functioning.
